Aleckna and Associates, LLC recognizes that transitioning clinician from paper to point of
care is a costly and resource intensive task. This transition requires the clinicians to
learn not only the technology but also a new form of clinical documentation. Developing
the plan to transition your existing "paper chart" to an automated clinical record, often
moving them from care plans to care paths, can be challenging. The following clinical
transition services are offered to assist you in meeting your objectives:
Analyze current clinical system development and existing clinical documentation
Review current clinical model
Assist in developing a clinical transition plan
Identify organizational events that may impact clinical roll-out/training
(i.e. PPS training, JCAHO or CHAP surveys) and modify clinical implementation to
integrate these events
Assist in the development and implementation of a competency based clinical training
program
Provide training resources as needed (see Training Assistance)
Assure appropriate policies and procedures are developed to support new processes and
achieve positive regulatory and accrediting surveys
